People Who Inject Drugs Model
The "Break the Cycle" (BTC) model was first introduced to the Central Asia region under the USAID Drug Demand Reduction Program in four countries where it was established as a regional best practice. BTC is an intervention which aims to reduce the number of people who begin injecting. The campaign works by targeting active people who inject drugs (PWID) and encouraging them to: reduce injecting in front of non-injectors (modelling); reduce discussion about injecting – especially about its benefits – with people who are at risk of trying it; refrain from helping non-injectors learn how to inject drugs; and develop skills for managing requests to give someone their first injection.
Under the USAID Dialogue on HIV and TB Project, the model has been expanded to include an emphasis on additional behaviours: routine HIV and TB testing, consistent condom use and reduction in sharing of injection equipment. The Project offers a targeted outreach package of services (TOPS) to PWID. TOPS is a comprehensive, multiple contact approach which provides detailed health information, distribution of commodities, health service referrals with escort when needed, follow-up to testing, and case management services for adherence support for those receiving TB treatment, ARV treatment and/or drug treatment.
